SOLVED. Dystopian(?), probably YA fiction. I remember the title and the cover for this book, but I can't seem to find it anywhere. [s]

I'll start by saying that I read very little of this book when I had it, so I remember nothing about the plot or characters. However, I know that I bought it somewhere around 2011 to 2013, and I distinctly remember the title and cover of this book. I think the genre was possibly dystopian, or sci-fi, and it may have been for young adults since it was pretty small. The title was two numbers, it was called "04" or "03" or something along those lines, and the cover was very greyish and depicted an arm with several thin tentacles growing from the back of the hand. Despite this, I've looked all over the internet for it but I can't seem to find anything relating to it. If you guys know anything at all about this seemingly-nonexistent book, anything you can share would be greatly appreciated.
