SOLVED. Romance Hero spurns heroine when she confesses to him. He is her best friend right before he is to leave for med school

Hi,
I am looking for a book I read maybe 5 years ago. It is about a girl who has been in love with her best friend and decides to confess right before he is to leave for school. His girlfriend had warned him that the friend had feelings for him. He gets mad and leaves, leaving the heroine alone at a bar ( I think). She gets attacked that night by the hero’s brother.
Years later the hero comes back to town and sees the heroine again and she has a son. He becomes close to them, and eventually the son says something about his dad that makes the hero realize it’s his brother. He confronts the heroine and she finally tells him what happened that night.

I just remembered that the hero's brother died not long after he attacked the heroine. I believe it was due to drugs.
