Also, question about different editions of the same book: What are the conditions that have to be met to combine different (particularly international) editions?

Basically if they are the same story, even if it's in a different language, then they get combined. About the only time we don't combine international editions is if they are split into multiple volumes when republished in another language/region and a single book in the other language/region.
