I read this book in the mid 90s but it could be a little older than that. It is about a Scottish laird/cheiftan who is being hidden by his own clan. Could be hiding from the English or a rival clan, don't remember. While in hiding the woman he is betrothed to shows up (I think she is English). They have never met and he is supposedly missing/presumed dead. He wears a disguise and pretends to be mentally challenged so he can spend time with her and get to know her without blowing his cover. He begins to fall for her meanwhile she does things like make puppets for him to play with believing he has the mentality of a small child. I remember the heroine is a bbw. I think there is some reference in the story about thistles and/or hawthornes.
