This is a book set in New Orleans. The main character is a detective with a birth defect, a small arm coming out of the center of his chest about 12 in Long that he doesn't really have control over. It kind of has a mind of its own. His partner is an Asian Elvis impersonator. There's one scene where the bad guys have guns on him and he has his hands up. And a little arm snakes out through his buttons on his shirt holding a derringer pistol.
