Need readers to read and take a survey on my story! 16yr+

So, I did a novelette for writing month, and I'm looking for some people to read a rough draft, who know and recognize it's a rough draft, then take a survey!

It is for 16 or older due to strong language

I'm not looking for grammar moderators, here. As I said before, it's a rough draft. I have already made some changes, but still would like input on questions.
