Created but could not use ISBN 13 as it is assigned to another book (Doc Voodoo: Crossfire).

I think the ISBN is incorrect for Doc Voodoo but I was unable to verify the other data for that ebook and so did not make any changes.

Yes, the ISBN is incorrect for the Doc Voodoo Crossfire e-book with that ISBN has been unpublished. Can we get it removed from Goodreads?

So the book was originally published as doc voodoo, then republished with the new name?

In that case they should be combined (either way, ISBNs shouldn't be reused, they are supposed to be edition unique)

GR keeps out of print book listings, and the first book it was issued to keeps it. You can put a librarian note on the other book to say it is a reused ISBN)

Actually ISBNs do get reused, there is a large number of instances where you will find this has happened. Often with the same book, but I have seen ones where it was a totally different book, but by the same publisher.

And the initial assignment was an error made through Smashwords, not Bowker or Book In Print. 978-1940761039 was assigned to "The Screaming Field" print edition through Bowker.

Was the mistake in ISBN13 corrected before or after it went on sale at Smashwords?

2 copies of Doc Voodoo Crossfire were sold with the wrong ISBN number ( 978-1940761039).

then it can't be deleted, sorry. and Goodreads policy in this instance is that the first book on Goodreads with an ISBN keeps it. i removed the ISBN13 from the reissued edition of The Screaming Field but added a note in the book description:
