SOLVED. YA Book dystopian ish where the characters probably lived on a city that floated above the ground? [s]

So I read this book probably around 2013/4 ish and unfortunately I don't remember any major plot points. The main character's best friend's name was Pen, but it was a nickname because there were multiple girls named Margaret (?? possibly her real name). Also her grandmother died of one of the last types of cancer left in their city, because things are now really controlled and kept perfect. Like, where your spouse is assigned I think and that kind of thing. There was also a royal family and I'm pretty sure the prince was going to be the love interest. I think that there was a bird on the front cover and that it might have had a black background
