SOLVED. 80's romance novel where the hero was in love with an indifferent heroine, had a gypsy Grandmother. [s]

I am trying to find a romance novel I read in the 1980s which featured a trope less traveled. It was set before the 20th century in America. The hero (whose name I don't remember though I remember his grandmother was a Roma [referred to as a Gypsy in the book] woman) was already in love with the heroine (whose name was Kate), had been for ages, but she loved someone else. On the night the someone else character marries Kate's stepsister, Kate and the hero are alone. Kate's unhinged. He confesses his love for her and they have sex. It results in a pregnancy that forces them to get married, but not before Kate tells the hero she despises him. The rest of the book is her realizing she does love him and then seeking his forgiveness. I believe Kate's stepmother tried to kill her toward the end of the book also. Of course, I can't remember the title, but I remember the cover of the hardback addition featured a pale woman's torso up to the neck, but no face and the woman was wearing a prominent necklace.
