Amanda, regarding the heroine's job - can we exclude physiotherapist and nurse?
What's the location (country/region/city/small town)? Around what year is the present time?
Can you tell us more about the hero - e.g., his current age, who lives in his house, his skills/interests/hobbies?

Story was set in either England or America. I want to say New England but again I don't remember exactly. Early 1900s, so 1900-1910 is the time period.
Hero is probably twenties, maybe early thirties. There were servants in the house but no other family members that I remember. Parents were probably dead and left him money. Aside from the piano playing I don't know of any other hobbies or interests.
The first chapter (or prologue) began with the hero getting a new car. He's never driven one before (cars were new inventions at the time) and his girlfriend/fiancee is nervous about riding with him. They get into an accident and it is implied she was killed.
The next chapter took place some time later (years?) and switched to the POV of a woman who was coming to his house (he was very wealthy) to answer a job advertisement. She gets hired as a servant or helper for the man, who was left crippled by the accident. He plays the piano but has difficulty doing it because of his injuries.
I didn't read any further than that, but it was listed as a romance so I'm assuming they fall in love or whatever.
