Yes, though it is now sold by ProQuest, the successor to UMI Dissertation Service, who produced the copy I got through interlibrary loan.
Then it sounds like it meets our criteria for being a published book.
However, the printing date should only be used as the publication date if the printing differs in some significant way (such as a different cover) from the original publication.

There is no ISBN but it is in book form. If so, are there any special considerations? Do I assume correctly that the date printed is the publishing date rather than the date the dissertation was submitted?
