book about appendictis and aliens

Read this book so long ago. Barely remember it. I think it was a ya book. It started off with the main character's best friend dying from appendicitis. All this weIrd stuff happens in between that I don't really remember. I think in the end it's revealed his friend didn't die but he turned into an alien. I also think this was happening to other kids when they hit puberty and it was some huge thing the parents of the town were trying to cover up. I kind of remember lightning or electricity being involved somehow but im not sure.

I don't remember the cover. It may have been one of the alien characters with lightning around him or something. That could just be my imagination.
