SOLVED. Adult Sci-Fi A man who is imprisoned on a moon/planet, in an idyllic paradise prison. The food makes the prisoners compliant, prisoners wear explosive collars around their neck.

This was an older sci-fi paperback I read in the early 2000s. The man gets captured/wakes up? in an idyllic paradise alongside numerous other prisoners. Everyone is very friendly but he gets the feeling that something is very wrong. The food makes your thoughts muddy and makes you compliant. I believe he eats fruit off a tree in the compound to keep his thoughts clear. The prisoners are all fitted with a collar around their necks that will explode if they escape.

They are apparently imprisoned there to be used as organ farms for the wealthy, I think.

He ends up escaping by hiding under bodies piled up outside the compound? and his companions get him aboard their ship where they perform an emergency surgery of incredible complexity with very low chances of success, removing his head to take off the collar before reattaching his head. Tampering with the collar in any way would cause it to go off, hence the drastic measures. The surgery is a success and he lives.
