YA distopian novel I read 20+years ago - Possibly called the citadel [s]

I've been trying to find a book i borrowed from the library when I was about 8 or 9. I think the book was called The Citadel and must have been written before 1990. I think the plot was about teens being kidnapped by rulers of the citadel and being forcibly given heroin.

I think the cover had a picture of a tower that resembled a syringe.

Not much to go o.

That could be it. It's been so long since I borrowed it that not much about it has stuck in my memory.

To think though I've been trying to find it for so long and it turns out that it's by one of the most prolific and well-known authors around.
