The h did seem surprisingly virginal for a bride in the 1970s (I mean, even in Norma Klein and Judy Blume, teens sometimes had sex with their boyfriends) but I was under the impression that Harlequins were like that in those days; i.e. anachronistically chaste.
I thought classic Harlequins had sex, but used silly euphemisms. Never read one from the '70s, though.You might want to check these lists to see if anything looks familiar....

Thanks, Rainbowheart, but I didn't see it in either of those lists. I got a little excited when I saw Be My Wife, but it turns out the man is scarred in that one, and it was not published until the 1990s anyway. But very similar in tone; I looked for more books by that author but it wasn't amongst them either.
Maybe something by Nora Sanderson? The Ordeal of Nurse Thompson has a heroine in an accident and The Two Faces of Nurse Roberts has a heroine with a scar, but it doesn't seem like they match up that well. I'm finding a lot of nurse fiction that mentions accidents and scars. The heroine wasn't a nurse, though, right?
I also found The Promise by Danielle Steel. It does involve a disfiguring car accident on a couple's wedding day, but it sounds like they are kept apart afterwards.

I'm fairly sure it was, if not a Harlequin, then a similar romance publisher, not a major writer like Danielle Steel.
And the plot was definitely (well, it has been nearly 40 years, but I'm 99% certain at least) that they were married but she refused to consummate the marriage, or even have close contact with him, because of the scar.

And I don't remember the h being a nurse (which seems to be Nora Sanderson's thing). If she had a job at all, it was something that a scarred person could escape to, like working with animals or small children.
Laird of Gaela and Carnival at San Cristobal have heroines who were scarred in car accidents, but the rest of the plots don't really seem to match up.Bump for your post, anyway.

No prob! Genre romance is not my forte, sadly. Have you tried posting in any of the romance groups? There are some people out there who are really experts in vintage romance.
I thought there might be, but I just went looking and didn't see any groups with a focus on vintage romance. It's mostly all newer erotica.I'll keep an eye out, and if I find one, I'll let you know.

The plot was about a young woman who, on her wedding night, is in some sort of accident. She is left with an almost invisible scar on her face, but she thinks she's now ugly, and refuses to consummate her marriage. Her husband is (understandably) frustrated and becomes friendly with a sympathetic woman.

As you might have guessed, her husband doesn't give a [hoot] about the tiny scar, the other woman is "just a friend" and not a threat to the marriage, and the bride and groom finally make love (off-screen).
