SOLVED. I *think* YA Fantasy, kids get recruited to go into dream worlds and they get stuck. [s]

I read it awhile ago and I can't remember the title. These kids go from dream to dream trying to figure out how to escape, and I kind of remember there being another kid that came up out of nowhere that same of them did't trust (I think she was a girl). And there was a scene where they were trying to steal some kind of bible for some reason. I remember the book being really long (maybe more than 1,000 pages) and really detailed, and also getting REALLY dark. There are some "restricted" dream worlds that only the adults are supposed to go into, but the kids end up in one of those worlds where there was some kind of disease and the ruler of the kingdom they were in drank children's blood to stay alive and there was a slaughterhouse for kids . . . Oh, and also, the main character would make things happen by staring hard at something and thinking what he wanted to happen. That was a major plot point but I can't remember why. :P Does anyone know what book I'm talking about? It's pretty unique so I'm betting someone out there know what it is....
