Bear Wiseman (drbearkele) My mom got me a choose-your-own adventure fantasy book from the (possibly Scholastic) book order form when I was in elementary school back in the 90s. I loved it, and I have no idea what happened to it. If I was to take a stab at when, I'd say the mid-late 90s, more likely mid-90s. I think those order forms had new or newish books, so I don't think it was an older book.

The main character or reader may have been sucked into a map to go on this adventure, or something like that.
I remember a wizard or something similar, with a riddle, "How many sooths does a soothsayer say on Friday" and the answer was "Forsooth."
I remember a character whose name was Lys or something along those lines. I knew it was a name I'd never heard and I wasn't sure how to pronounce it. She may have been a witch or a hag, or an enchantress who seemed to be a hag.
There was a foggy bridge, possibly with a knight nearby, that you could choose to cross or maybe follow the knight on another route. I know that one option led to a failed adventure or death of some sort.

If anyone knows it, I'd be extremely grateful! Thanks!

I recognized the cover art immediately - that's definitely it. Thanks so much! I think Egbert was the knight by the bridge.
