I read this 3-4 years ago, it's definitely not an old book. The story starts with a young guy going to a small island. I'm not sure what he was doing there, but he may have gotten sick while he was there.
In each short story, the soul mates were in different places in history. The one I remember more than the others was them being young viking children. The girl was sacrificed or sacrificed herself to save the boy from an evil being or person. At least one of them always died in each short story.
At the end of the book, the modern day islanders were trying to kill the pair, but I believe they may have gotten away.

This doesn't exactly match what you described, but it sounds similar to Midwinterblood by Marcus Sedgwick. It's made up of several short stories about two people who I guess you could call soul mates - they're connected in each story, but in different ways. It begins with a guy going to an island. And it ends similarly to what you're saying - I don't remember the details anymore, but there's something to do with the island community trying to sacrifice the man.
