My confusion is that most swings are not inside, yet most are small enough to be inside.

Yes, that's fine. You could technically have a swing inside

Yes, as long as they are depicted as separate objects, not as part of a bird. So the cover you posted works

The Tie That Binds by Kent Haruf The Tie That Binds by Kent Haruf and The Hopefuls The Hopefuls by Jennifer Close

There is a tie on the cover of The Hopefuls, but I don't think it is an actual Tie as an object that is being referred to in The Tie That Binds so I wanted to make sure it was ok for this task.

Yep, you're allowed to use words that have alternative meanings if spelled the same

Hi Shelia, the issue isn't the picture of the road, it's that you can't use the word 'road' as an object.

Just the shape of a heart wouldn't, but if it was an actual heart that had been removed from a body that would be OK.
