My money is on Rachel.

I think Kisten was made undead by whoever Piscary gave his last blood too and when Kisten lost his soul, he attacked Rachel who killed him in self-defense.

I think Trent is involved here somehow and had Quen alter Rachel's memory (like he was going to earlier).

I like your theory. Rachel was my guess but I had nothing other than a gut feeling. If Ivy had done it, she'd remember.
