No, sorry Sunshine - it would have to be D and S and they are way further apart than just 4 letters. The initial doesn't count. It has to be first and last name... (The initial H is the fifth letter with D as the first, so it wouldn't have worked anyway. My Kevin Hearne example was at the maximum length apart.)

If a book has two authors, do both authors have to fit, or can it qualify on just one? For example: Dead Ringer by Heidi Belleau and Sam Schooler.

One author is fine. So long as one fits the rule, it will qualify.

You can't mix the authors to fit the letters. It must the same person's initials used to fit the rule. ie only Sam Schooler; not Heidi Schooler (not that this works for rule, but hopefully you get my gist!)

Hi Ezi - Kristen Proby? That's 6 letters apart so it won't work :(

You have to count the K as the first letter and the P would be the 6th letter.

Is it on the MMR list for books set in countries outside the US? If it is there it will work. Otherwise you'll have to read it and see if it fits.

After the next theme is called, you can no longer change books on the previous week(s). You can only change your mind until the next theme is called. After that, I believe you are SOL.

Yes that's right. You can only change your book selection during the week the theme was called... Once the new theme is called, the book selection can't change.
