The author's website (unless it is a specific subpage for the book) should not be used. That is, www.jsmith.com should not be used, but www.jsmith.com/bookname would be fine. And some books have their own websites, which are also fine.

It's an acceptable option, yes. However, the URL is not required, so if there is no appropriate one, just leave it blank. And only add a URL if the link provides additional book information (excerpts, extras, and so on). Otherwise, best to leave the field blank.
