SOLVED. MG book set in Toronto about girl with younger brothers with autism [s]

I bought and read the book sometime between 2003 - 2005 in a bag with another random book (the other book was published in 2001), I don't remember the title or author and it was lost in a flood.

I believe it takes place in Toronto. It's about a girl who refuses to go by or reveal her real name, and prefers to be called by the name of a place she picked out from an atlas. It's hinted at that her mother was either (formerly or currently) a stripper or exotic dancer. She had two younger twin brothers (I believe they were twins) who were severely autistic, I think. A social worker stops by a lot to check on them, and always insists on calling the protagonist by her real name. The boys are almost school or pre-school age in the book, and the mother is thinking about sending them away (possibly at the insistence of the social worker) to some kind of special home / school thing for autistic children. In the end (view spoiler)[ she decides to send them to the home / school thing, but her and her daughter are going to move near them so they can see them all the time still. (hide spoiler)]
