Issue with Author name when defaults in

I am trying to add John Newton Sarber and Sarber County, Arkansas to my Author page. When my name defaults in it is first showing Frances Hodges then my full name which is Mary Frances Hodges and is then repeated several times. Because of this issue, I am not allowed to add it to my page. How do I correct this?

I've fixed the author name on these records and added the Kindle, but it looks like the bad version of your name came directly from ingram, the major book service where we get a lot of our data. You might want to check with your publisher to make sure they're sending out the right information.
