It featured a silver-haired girl who became the first female wizard. She lived in a world where people had their fates woven as a tapestry at birth and they had to complete it before they died. The only name I recall is that she had a winged insect-like mount named Opal-Fire. It started with her tricking Death into taking a woman's severed hand instead of the woman's life.

In one of the later books she is pregnant, and I believe her lover is a prince or nobleman of very high rank. And there's a dragon.
