How often do you give away a free copy of your book with promises that you'll get a review and then get nothing?

I'll review your books on my blog, if they fall under the category that we blog about (typically YA/middle grade).

I hope they weren't paperbacks to a blogger. That is an expense to us.I can't answer - I'm in the waiting stage with three bloggers (whom I do trust). I've given frees to readers, but not in exchange for a review. If they were just readers... I think readers review or not at their own sweet whim, and more can't be asked of them. If you had an arrangement, eg. through a group here, I guess that's different.
It can take months, though, I know that. Bloggers often indicate on their site what a backlog they have.
Fortunately that's never happened to me - well, only with one ebook version - though Bryn's right and reviews can take a long time to filter through.Unfortunately I think it's one of the risks we take.
I've had a few bloggers promise reviews and then nothing. On here, there have been a few who have received copies and nada. Fortunately, most people do eventually leave a review or at least a rating.

It's a numbers game. First, once I had at least 8 reviews on my Amazon site, more reviewers agreed. It's a numbers game. Out of 8 who agreed, I'll get one or two who'll commit to a review. Also, I did a Goodreads giveaway. I included a letter asking them to kindly leave a review and all of the six did so. The letter encouraged them to leave a review.
Sadly it does seem to be the norm. I've also done a couple of Goodreads giveaways. I sent signed copies with a letter inside asking if once they read the book they might be willing to review, or at least rate it. However no review was forthcoming. I guess they hated the books. :)
