Need help Combining Print/Kindle version of book

Could someone please combine the print version and the Kindle version of this book for me? Here are links for the two versions:

The Kindle version also is missing the author's middle initial. There also appears to be two pages of the paperback. I hope this makes sense to someone because I am utterly confused. Thanks for any help.

Ok, I've fixed the author's name, combined with the paperback and merged the two paperback records (one had no adds, so it wasn't a problem).

Record should be correct now.
