Also, Was it the fight between James and Cade that leaves Cade with brain injury or did something happen when Cade met Ivan? Wasn't very clear to me.

I think after Cade and James have the fight on the boat, maybe the waiter (in the bow tie) further harmed him? Cade did freak out when one of the professionals at the facility was wearing a bow tie. The waiter seems unlikely though. I didn't even think about what happened to the dog - good comment. James' hostile attitude also seemed a little too easily resolved. Stroke of luck though that Cade gets millions at the end and him and Kailey can "retire" to France. A little too "fairy tale". I hope author writes follow-up and gives Ryan a love story. and in general, I think there can be more than one "true" love.
