My opinion, and I didn't read too closely, is that Wolfheart is what he is called in fairy tales only. The woman in the black skirt seems surprised when Elsa refers to him by this name, as if the woman in the black skirt had never heard it before.
Side note: It bothers me that Elsa has a vocabulary so far beyond any imaginable 7 year old and yet still says the woman in the black skirt is a "terropist."

I'm 2/3 of the way through and I think it is just Wolfheart. The only other reference so far is pg. 257 "Sam's friend"
