I don't really get why it would be backwards - I actually think the title wasn't very fitting for the book. The "good girl" reference is in there a few times but I don't really think it played out throughout the book enough to be the title.

I think it refers to the fact that there's a complete reversal in the expectation from the "good girl" after reading the very last line, hence a reverse R.
