When beginning a novel, do you work out your plot first or do you just start writing and let the story unfold?

Okay, this is a really old question. Sorry about the delay. I'm fairly new to Goodreads, and my email isn't sure how to handle incoming inquiries.
The answer is sort of a hybrid. I know the beginning, a couple of points in the middle, and what I think is going to be the end. As the writing unfolds, I make new choices all the time and pray that I'm not gong to screw myself up at the end.

In book 4 of the Jonathan Grave series I was puzzled by the shortness of narrative concerning Gail. The ending particularly seemed almost an afterthought. What was in your mind here? Don't get me wrong I really enjoyed the book and am already into the next one, but just found this a bit strange.
