Crazy dreams on occasion begin the process. Most of the time, though, it's more about "I want to see where the 'What If' goes". There are so many books and movies/tv that go in one direction and then stop when they get to a satisfactory place. I always want to know what happens after the fact. It all begins with a quandry.
