As for what I do when I can't quite get something I'm trying to write right, that's usually a signal telling me that I may need to re-imagine the scene or plot point I'm stuck on. I make a list of all the things that could be different about the scene ("maybe everyone dies!" is usually at the top of the list, just to free me up to think big) and usually that helps me strike on a fix.
