My take on this was that by ensuring Vanessa climaxed throughout the rape, he could further manipulate her into believing it must have been consensual. I've read before that this is quite common with survivors who take on responsibility for their abuse because their body reacted "positively" in that one way.

I don't think Vanessa could orgasm through penetrative sex so it was easier for Strane to control her with oral sex. Moreover, he had Viagra so maybe he wasn't always capable of penetration, especially as both he and Vanessa grew older.
