Here's my take on that plot hole:

Before Shade/Ignitex open the box and restart time-he and Nyx have the following conversation

"But you won't die," I whispered
"And once time is unwound, neither will your mother..."
"Then I won't be born."
" I saw your father when he was desperate...I'm sure he'll think of something. Maybe it will be a better plan this time..."

The likelihood of the twins being born in the alternate Arcadia is slim... however taking into consideration this is a fairytale world, its probably safe to assume that destiny, a Lar, or magic played a role.

Going the destiny route, we'd have to assume that everybody was destined to be born no matter what Arcadia version we are in (Nyx seems assume this herself when she thinks about the new Arcadia where she imagines those same people that died, still living) ...however HOW they are born and even how they die may have many possibilities. The fact that the twins' mother, STILL ends up dying when the twins are young and their father still ends up with their aunt may help support this theory as well as result in Nyx and Lux ending up together at the end.

Second option is that a Lar played a role -specifically the Sparrow Lar that Nyx showed kindness to. He shows up at the end and convinces the Kindly Ones to take Nyx's deal...its possible that he may have also played a role in making sure that everything went right so that the twins were reborn AND that Nyx retained/regained her past life memories.

Last but not least if the twins' father was willing to take alternate measures for children by approaching a demon in the sundered Arcadia, its possible he found a magic related way for his wife to have children and survive. It is hard to say whether the route he took this time had any consequences since the mother dies sometime after the twins birth.

I'd especially like the Lar theory, but it could also be a mix of the three. I guess its up to interpretation of the conversation between Nyx and Lux have before time is reset as well as the actual ending.

Alianoraree The answer to that isn't stated, but there are lots of possibilities. Remember, it's been nine hundred years since the Prince was taken; a time in which the real sky showed, there was no Gentle Lord, and there would probably have been fewer demons. (They leaked into the island's world so much because it was inside the box with lots of trapped demons.) Who knows how history would have been different?
Maybe one of the Resurgandi's experiments aimed at finding how to destroy the Gentle Lord had the side effect of making Nyx's father infertile.
Maybe Nyx's mother was infertile as a consequence of a deal someone had made with the Gentle Lord in the past. (For instance, she could have bargained away the chance for children but then deeply regretted it later. Or there could have been a deal by some ancestor that he'd get nine generations of children but then his line would die out.)
Nyx might even have different ancestors, for all that her culture and family situation seem pretty similar. If the final prince failing the box-test meant the Kindly Ones were freed of their obligation to protect the country from invaders, maybe the "barbarians" did invade, and the change in genes meant Nyx's parents were more fertile.
With the real sky showing, maybe the weather and climate were different enough that more or different foods thrived. Diet affects health, which affects the ability to conceive and carry a child to term.
Perhaps the most likely theory is that Nyx's mother would soon have become pregnant even if Nyx's father hadn't made the deal. Seems to me that would fit the Kindly Ones' malicious sense of humor.

Rocky Although I am way late to comment on this. The reason Nix and her sister are alive could be because their parents would have had twins anyway. It could be that they did not need the bargain to have twins.

He It said in the book that her father would have found a way to have children through some other means than the Gentle Lord. So we have to assume that he either made a bargain with the Kindly Ones directly, some other agent of their power, or even some other god/being.
