Question (and I may have missed this) - who was Candice Addis? Should we have known her?

No, Candice Addis wasn't a character in the book until Saint visits her at the end. All we know is she was Theodore's adoptive mother, & Saint "remembers her from the hospital." She could have been a nurse (abortion clinic, ER, maternity ward?), but more likely they met when she came to the hospital to pick up the baby.
