I usually recommend Thorn for ages 15+ and would add a CN for abuse, assault, and animal and character death. It's clean in the sense that there isn't any "on screen" sex or cussing, but the content in this book is heavy. There are multiple mentions of abuse (physical, mental, and sexual), one character is r*ped and left for dead, characters are tortured and executed. Maybe a mature middle schooler could read it, but it's really up to reader discretion. So in a sense yes it's clean because there isn't any sex done by the MCs, but no it's not because of how dark and heavy the content is. (less)

The issue is really just assuring the reader has the emotional maturity to deal with what they're reading--so there may be particular younger readers who will do fine, and there may be older readers who won't, but 15 seems like the best guideline I can identify.
