Sherlock is definitely being flirty/ ‘oh aren’t I so gosh darn irrascible’. John, however, to be true to thecontext, when he just discovered he was NOT actually going to die at any second and Sherlock had known that, is thinking about how he wants to kill Sherlock. 

I found this scene very uncomfortable, like Sherlock was genuinely nearly unforgivable rather than being funny or charming.
