How to Write a Query Letter

I'm shopping around for an agent so I found one that accepts email queries, but their website didn't indicate how they wanted the query letter structured.  I found www.agentquery.com and wrote mine based on their suggestions.  I've also included all that info under the 'promoting a book' link.
