Theory based on Moffat quote:
"The 12 regenerations limit is a central part of Doctor Who mythology - science fiction is all about rules, you can’t just casually break them."

I think that the person/soul that is the current Doctor (Smith back through Hartnell) can’t regenerate anymore. 

I believe he will retire (go home?) and become The Curator. So, he met himself not as The Doctor, but as The Curator at the end of Day of the Doctor. This both explains why Baker says “Maybe I was you.” and has aged markedly…
He will pass on the name of The Doctor and its commensurate duties to Capaldi, ushering in a new Time of the Doctor (the title of the Christmas Special)…

Perhaps, Capaldi will be a “loomed” (http://tardis.wikia.com/wiki/Loom) Lungbarrow, born as an adult. Because Smith can’t find Gallifrey proper in time to do anything about dying. 



 ”[Orla] Brady’s character is someone from the Doctor’s past, with the plot of the festive episode revolving around her.” Perhaps the actress in Time of the Doctor is a displaced Gallifreyan (not TimeLady) that helps him with the looming..



A new addition to this thought process:
Just before he steps into his TARDIS to leave in Day of the Doctor, David Tennant says, “We need to find another destination, because I don’t want to go.” While I understand that the second half of that is a nod to his regeneration, it’s the first half that comes into play here.
He doesn’t want to go to Trenzalore. He doesn’t want to die. So by “retiring” and picking up the name of The Curator, he doesn’t have to



Everyone seems to forget that River Song gave the Doctor all of her remaining regenerations (which may or may not have been the same as a Timelord).
