form fw7, getting the pesky ITIN

It's going off in the mail today. I had help filling it. Author Angela Stone left a blog post with exact details.

So at last, I have this envelope on the desk with a filled form, a letter from Smashwords and my frickin' passport. I'm not photogenic. I lost count of the retake photos to get this passport done.
