Let’s articulate it.  Look at his mouth when he pronounced the “sh” in Sherlock:

But here’s its shape right before he spoke, at the end of his inhale:

Why open it so wide?  He stood there with his mouth shut for long enough:

so it’s not just about nervous breathing.  To answer the question, let’s examine Sherlock’s mouth starting to form some words that do require this shape:

So whatever he was about to say, it likely began with “I”.  Now what could that possibly be?  Hmmm…
