Catalyst is the sequel. Or at least a semi-sequel.

Gail Cohen | 1 comments The author also wrote another book called Twisted. It's theme is similar to speak, but the main character is a boy in high school.

There is no sequel and Catalyst isn't even a semi-sequel, just set in the same school. Twisted isn't either. Melinda does make a cameo, but other than that there's no relation. Laurie Halse Anderson talked about making a sequel, but so far nothing has arises about that being in motion.
