Making Problems Visible

Recently while reading this book, we came across the section in on Making Problem Visible, while this seems easy in theory. The demands of the work day make it hard to stop, write down the problem, then solve at a later point thus leading to a band aid fix. Is there a simple solution or recommendation out there that can be applied?
Would ensuring that we have enough time at the end of every day to reflect on our day help ensure we can bring the problems to the forefront?
