Vintage: A Ghost Story - I believe the MC's first name is never mentioned. And I love the book, so it's not an obstacle, as long as it doesn't become self-consciously deliberate. To be honest, I never even noticed when I first read it. But I'm not good with names, and don't care much about physical descriptions. If I have a good gestalt for the character (in this case first person narrator) I don't need the name. If it gets confusing or there are places where the name would be expected and they're deliberately avoided, then it's done wrong.

They even have a thread where writers go in and ask for cover suggestions. Some of the covers look awesome. https://www.goodreads.com/topic/show/...

Note: Their covers are NOT limited to YA but everything I saw was suitable for general consumption.
