Co-author missing and edition merge needed

My name somehow got removed from the author information, and Matt's is listed twice, instead. Could someone please re-add Eric Zawadzki as author to both of these?

Also, there are two editions (ebook and paperback), but the ebook version doesn't have a cover. Would someone be willing to merge these editions?

I fixed the author info.

The editions are already combined. We only "merge" (delete) true duplicates. That is a distinct edition, with its own ASIN. We don't merge those.
