Is this a young adult or adult book? I plugged in a search and this is what came up. The Dark Hills Divide Hope you find it. It sounds interesting!

The Darwath series by Barbara Hambly? Ie The Walls of Air There's an ice age coming, and cold loving monsters, and the remaining human kingdom retreats to an old fortress.
No, that's not it. But that does sound good. I may have to read that one too.

What I remember about the book is there is a walled city. Supposed criminals and their ancestors live outside the walls. I remember there being a ceremony for those outside the walls to be reinstated as a citizen of the city. When the city is attacked (?) the survivors make a run for it and end up in a cave. I also remember a boy from outside the walls being "special" in some way.
