How do I combine two listings of my book?

My book Connected is listed twice - once as an eBook and once as a paperback. Both have unique adds, comments, and ratings so I don't want to merge them. Through reading the GR manual, it's my understanding I need to have a librarian combine them instead. Is this something someone has to do for me, or can I get instructions somewhere? Thank you!

I've done it for you Jolyn, not sure if you can do it yourself, some Authors have Librarian status, some don't.
