I haven't been listing miniseries issues individually when the whole miniseries has been collected only in one book; it seems to me that "comics miniseries" and "one book" become indistinguishable at that point....

I'll be honest, I don't know about too many comics. I've only just started reading them again, but I'll add the ones I get a chance to look at :)
I'm not sure if these should also be listed as single issues. Vol. 1 collects the 3 "Those Left Behind" issues. Vol. 2 collects the 3 "Better Days" issues and The Shepherd's Tale was not published individually...
