Fixed.

I think the problem somehow caused by the "3rd party" image from amazon.com
I've found several similar "funny" problem. It is easily fixed by uploading a cover art.
If I didn't have one for the book, I usually just save the available picture and then uploading it manually.
Not sure about the copyright though.

Can anybody clarify about the copyright issue?
